One of our friendly team members will greet you as you walk in the door, take your ID and insurance card (if applicable), and get you set up with our new patient intake form if you haven't already filled out prior to arrival. Once you have completed your intake form, we will quickly bring you back to a private room while our team reviews your intake form.
